We sent our medicals somewhere around April or June. I got the CAIPS done now, and they show that my husband's medical wasn't received. So we have got it done again, and want to send it. The doctor says that the address has changed, but he doesn't know the new one.
Please this is urgent, does anybody know the new address of OTTAWA where the medicals need to be sent? Please respond if you know or have undergone medical exam recently.
Since we photocopied the blank form when it came, and now the doc is filling that one up. Is this the right way to send the medical again?
