Need sample covering letters for EAD & AP

My EADs due for first renewal...Last time My Attorney did it, He is asking additional charge for renewals...SI I am planning to do it for my self this time
do anyone havea any Sample covering letters for EAD and 131 (AP)..... can any share sample letter
