Need information on form 485

I work and live in Missouri. My employer filed labor in IL and got approved. I am filing 140/485 concurrent. I have couple of questions needs to be answered.

1. Do I have to send I-485 along with I-140 or do I have to send both separately and where do I send?
2. What should I select in form I-485 section II?
