Need help with self filing of AP

Dear Knowledgeable users,
I have a EB2-NIW approved petition, with pending I485 (lost in retrogression mess). Last paroled in to US in March 2008

I have an AP approved (thro a lawyer) and valid till Jul 1 2009.
I was trying to apply AP by myself this time and had following questions.

1) Part 1 column 3: Class of admission
Is the answer, Parolee?

2) Re required doumentation issued by USCIS showing your present status in US
Does it mean I-94 given to me on my last entry?

3) Is there any place in current form to indicate this is for renewal of prior AP. i could not find one. Should i then indicate in a cover letter? What else the cover letter should indicate?

4) Part 3: Date of intended departure and length of stay
If i do not have a definite date in mind, What do I enter?

Kindly take time to answer my relatively simple queries

1) Yes, "Parolee" or "Parolee, AOS pending".

2) In your case, your I-485 receipt notice is actually your current evidence of status, not your I-94, although you could send a copy of that too.

3) No, there is technically no "AP renewal". You just apply for a new one when you need it. I'm sure you are aware that you are not required to have one, it's only if you anticipate travel or emergency travel. I always kept one in case there is an emergency with family abroad, but no, there is no "renewal" checkbox.

4) I always made something up around holiday dates - they don't really care. You can also write "N/A" since the AP is technically for emergencies and you can't anticipate those. Again, they won't care what's written there.
