Need help with filling I-90 form

Hello Friends,

I'm helping my mother to fill up an application to replace Permanent Resident card which is going to expire in few months. I have a little confusion about two questions which they have under Part 3 "Processing information"...

1 Q. City of Residence where you have applied for an Immigrant Visa or Adjustment of Status.

My mother came on Immigrant Visa with yellow package and got stamp on her Indian passport for legal permanent residence and received her Green Card in couple of months in mail. She had an interview in Delhi, India consulate, so should we put Delhi for an answer?

2. Q Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted.

She got her yellow package or Visa from Delhi so I assume the answer is Delhi, India?

Please help. Thanks

That is exactly what I need help with. I was helping my dad filling up the form and I got stuck between these 2 questions.

One more thing:

On Part 2,

It says Application Type

for 1. My status is

A Permanent resident ( not a commuter )
b Permanent resident ( commuter)
C Conditional Permanent Resident

First of all I did not mark any of them because my mom never received her green card, so should I leave it blank? Please reply thanks. And I guess I would put it same way like you did on those 2 questions, I would put it where she applied in India I guess. Thanks
