After I-140 approval for my husband, we are now filing I-485 and attorney requested aff. of support from applicant to spouse and children. Which is the right form to file? there are several (I-864/ I-134)

I think it is I134. I just filed my 485. The primary applicant (me) does not need it since it is employment based. For family members my lawyer filled out the form (for each) and I just sign it. I thought every lawyer is also a public notary. If your lawyer is not, you may have to get it notarized somewhere else.
