Date of Intended Departure

How accurate should this be? I do not plan the exact date of departure yet.

If you are applying for an Advance Parole Document, skip to Part 7.

Since I am applying for an Advanced Parole Document. I should not enter eny information on sections 4,5 and 6. Just double checking.

On a separate sheet(s) of paper, please explain how you qualify for an advance parole document and what circumstances warrant issuance of advance parole. Include copies of any documents you wish considered.

I want to visit my family in Turkey with my wife. Can I write this on as a valid reason?

1. For how many trips do you intend to use this document?

I would like to choose multiple. Does it hurt?

2. If the person intended to receive an advance parole document is outside the United States, provide the location (city and country) of the American embassy or consulate or the DHS overseas office that you want us to notify.
3. If the travel document will be delivered to an overseas office, where should the notice to pick up the document be sent:

I will not enter any information on these two since it does not apply to my case. Is this correct?

11. Have you ever before applied for employment authorization from USCIS?

I am working with CPT right now. Should I check "Yes" for this question? If Yes, I do not know the answer to "Which USCIS Office?". What can I do about it?

Date, City and Country of Birth (If known)

My wife's father left them when they were little. She does not know when he was born. She knows where he lives, but does not want to contact him. If we leave the date of birth (for the father) field empty, is it OK?

City and Country of Residence

My wife's mother passed away. Can I type deceased into this field?

21. If filing for your husband/wife, give last address at which you lived together.

We are still living together. What should I enter to the To (Date) field? Current or Present? Or is there a better alternative?

23. My current individual annual income is:

I am on F-1 visa and working with CPT. Last year we field together. Should we enter the total amount (me an my wife made) or just my wife's income to this field? If we enter the total should I also fill an I-864? Her income is like 28K and is over the minimum. I do not want to complicate things.

Also I have the Tax Return Transcripts from the IRS. I see a number 1040 on top of the paper (not 1040EZ). I also have the form 1040A which we got when we filed our taxes. Which documents should I submit with I864.

For initial evidence (bonafide marriages)
I am planning to send these in:
- Marriage certificate
- Our last 2 leases that shows we lived together
- A credit card statement that shows our names on it
- We have two joint accounts in Citibank. I am planning to talk with the bank and get some kind of document showing this.

Do you think these are enough?

I know this has been a long post :o Thanks in advance.

Date of Intended Departure

How accurate should this be? I do not plan the exact date of departure yet Just put something, around 1 month before you intend to leave. I would say at least 45 days from todays date, maybe they will hurry up and process it faster.

If you are applying for an Advance Parole Document, skip to Part 7.

Since I am applying for an Advanced Parole Document. I should not enter eny information on sections 4,5 and 6. Just double checking. That is correct, you do not need to put anything in those sections

On a separate sheet(s) of paper, please explain how you qualify for an advance parole document and what circumstances warrant issuance of advance parole. Include copies of any documents you wish considered.

I want to visit my family in Turkey with my wife. Can I write this on as a valid reason? You do not need to add anything, you are entitled to have that document because you are adjusting status. Do not include anything

1. For how many trips do you intend to use this document?

I would like to choose multiple. Does it hurt? Nope. I did and got 2 copies, some people get 3... either way, you are fine.

2. If the person intended to receive an advance parole document is outside the United States, provide the location (city and country) of the American embassy or consulate or the DHS overseas office that you want us to notify.
3. If the travel document will be delivered to an overseas office, where should the notice to pick up the document be sent:

11. Have you ever before applied for employment authorization from USCIS?

I am working with CPT right now. Should I check "Yes" for this question? If Yes, I do not know the answer to "Which USCIS Office?". What can I do about it? Well, what city did you sent your application for OPT to?

My wife's father left them when they were little. She does not know when he was born. She knows where he lives, but does not want to contact him. If we leave the date of birth (for the father) field empty, is it OK? Can you guess-timate it? if not then put in unknown

City and Country of Residence

My wife's mother passed away. Can I type deceased into this field? They need to know her country of residency, so put it, if they ask later then say she died.

If we enter the total should I also fill an I-864? Her income is like 28K and is over the minimum. I do not want to complicate things. She is sponsoring you, you do not need to file an I-864 for yourself, but yeah, include both of your incomes.

Also I have the Tax Return Transcripts from the IRS. I see a number 1040 on top of the paper (not 1040EZ). I also have the form 1040A which we got when we filed our taxes. Which documents should I submit with I864. Submit the Return Transcripts, that should be enough.

- We have two joint accounts in Citibank. I am planning to talk with the bank and get some kind of document showing this. Do you get a bank statement with both of your names? that is enough.

Do you think these are enough? You need as much evidence as you can. Utility bills? Car titles? anything with both of your names.

I know this has been a long post :o Thanks in advance.
