Need help to fill-out Form I-90

I am in the process of helping out a friend fill out the Form I-90, and came across some questions that he is confused about. So, I am posting here hoping to get some help.
In Part# 3. Processing information
Q1. City of Residence where you applied for an immigrant Visa or Adjustment of Status? He was living and residing in city X, but had to get the immigrant visa interview and visa stamp in city Y, so what should be the correct answer here?
Q2. Date of Admission as an immigrant or Adjustment of Status? Here is the catch, he apparently did not realize that his green card was issued with incorrect information because of a USIC administrative error, as the GC had his date of birth in the section "Residence Since" instead of the actual date. So, my question is what is the actual date of Admission? Is it the date that his immigrant visa was issued on or the date of his first entry into the U.S.A with his immigrant visa or the date the GC was issued on. It is worth nothing that the expiration date of GC is showing a 10 years duration from the date the GC was issued and not from the date of his first entry into the U.S.A.
Q3. It has been one and half years since he got the GC with the incorrect info, so does he have to pay for the fees and biometrics to get a new GC with the correct info or not?
Q4. What documents other than the original issued GC need to be included?
