Need help: Question Form I-130 (visa/I-94)

Hello, I'd like to ask a few questions regarding I-94. My fiance (US citizen) and I are getting married soon and he is petitioning for me.

My situation:
My last entry into the USA was on January 3, 2003. On my last I-94, it was stamped and written that I was admitted until June 11, 2004. I have not left the USA since. I have also been granted extension on my H1-B1 visa with an expiration date on June 1, 2007.
My questions :
1. On I-130, what do I answer on “Date authorized stay expired, or will expire, as shown on Form I-94 or I-95?” Would it be June 11, 2004 or June 1, 2007?
2. Is the I-94 number the number on the upper left-hand corner of I-94 form that is hand-written? Is I-94 always the same every time I enter and exit USA?
3. On the form I-130, what would my fiance put under “This form is submitted in connection with application for?” Would it be “Status as Permanent” or does he have to explain that he is petitioning for me?

If your H1B extension has been approved, you would get a NOA, (approval notice) for your H1B extension. The new I-94 with the extended dates would be attached at the bottom of the approval notice. You should be using the date from your new I-94.

2. Is the I-94 number the number on the upper left-hand corner of I-94 form that is hand-written? Is I-94 always the same every time I enter and exit USA?

I dont have I-94 in front of me here, so ill tell you what i remember of it. There would be a 11 digit number printed (not hand written) on the top left. You should use that number. Your I-94 form would be same everytime you enter US, but number would be different.

3. On the form I-130, what would my fiance put under “This form is submitted in connection with application for?” Would it be “Status as Permanent” or does he have to explain that he is petitioning for me?

Status as Permanent resident. Also, if you want your fiance could check "Other" and explain what the form is for, but usually not needed.
