Need help in filling out NIV form for revalidation!!!!

Before I post my question, I would like to thank RK for maintaining such a wonderful site. I found it really useful.
Ok, here is my confusion. I am applying for H-1 revalidation. There are few questions in the NIV form like 1) purpose of visit to USA 2) When do you intend to arrive in the USA. So, what to answer for those things. Shall I leave them blank or write something like N/A. Help me out. Thanks again.

I would strongly suggest to speak with your immigration attorney for the questions you have. Anyway, this is what I did and got approved...
1) Purpose of the trip : Business
2) When do you intend to arrive in US? : "Currently living in USA"(if you\'re already in USA).

Once again, please talk to your immigration attroney regarding this.
