i already submitted with I-485 originally.
3- I-864 with your tax returns for the last three years,

4- Your employment authorization card
5- proof of legal entry in to U.S., Passport, or I-485 supplement A

I assume this is your I-485 interview. Then, you are the petitioner and the beneficiary. This is nothing to do with your employer. If this is your spouse's interview, then I think you are the petitioner and your spouse is the beneficiary.

For the medical, they must've misplaced your I-693. Ask your doctor to re-produce the form. This shouldn't be too hard.
