Need Help - 3rd Country Visa

Does anyone have experience going to US consulate in Mexico to apply for a visa? I appreciate your expert help on the following questions:

1. If you're approved, do you get the visa on the same day? Or you have to wait in mexico several days to get the visa? The answer will help me plan the trip.

2. My wife and I don’t have our original I-94s any more because we changed status from F-1 (F-2) to H1 (H4) from our original entry about 10 years ago. Is the new blank I-94 attached to our recent approval letters enough to leave and re-enter US?

3. My wife got denied H4 before because I didn’t apply for her timely. Then later we appealed and get re-approved. Could my wife get a 10-year bar because she overstayed more than 365 days? If yes, how does the officer at the port know that she overstayed? Because she was never in any removal proceedings and no one asked her to leave US.
