I have a double names like shiva and siva, can i take affidavit for my LC and GC process and also please let me know is there any standard affidavit format is avilable? or please let me know the other options also.Please advice me.

Not sure if can get away by a mere submission of Affidavit. But here you go.. If I were you, I would consider getting your LC (and I-140/485 if applicable) amended to match the one stated in your passport.
