need affidavit for H4 ??

Hello all...

I heard that one might need an affidavit saying that you are sponsoring your spouse for dependant visa(H4). There is some form which INS provides, that you can fill and get notarized. Could anyone please help me out here? Where can I get such a form?

Thanks for ur help, But its seems that it is required for Immigration-Visa. I was concerned that does Non-Immigrant dependant visa(H4) also need it? And if so, is it MUST? Could you please advise me on this?

INS Form I-134, Affidavit of Support, sound more appropriate in this situation - can anyone confirm that ?
