Need advise on items 3 and 5 of G-731C form

Hi gurus,

My I-485 petition was approved on September 13, 2004 (CSC processing). I stamped the passport on September 29, 2004 (Sacramento, California). I guess if I do not receive the physical green card by some time in december I probably should file the form G-731 with the service center
(CSC in my case). If you are familiar with this form could you advise me please on how to fill the following two items:

3. Port of Entry/Office

5. Date of Entry of Adjustment
