Naturalization Last Name Change & OCI

My wife changed her last name to my last name during naturalization and she was only given the naturalization certificate with the new last name during the ceremony. There was no judge involved and no name change certificate was given.

When applying for OCI does she need to send a affadavit or advertise in newspapers? I tried to search the forums but did not get an answer. Experts please advise.

You can apply for adult name change in the probate court where you live so you have an official name change document. I posted the details on how to get one earlier. Below is the link

Hi Await_GC,
Thanks for the response but i am trying to avoid a name change at the county court. I am aware of the procedure and it costs approx $450 in FL for a name change. I am not sure if a notarized affadavit will do it otherwise I will advertise in news papers.

For marriage based name change (last and sometimes middle), you may want to supply marriage certificate and that should take care of it. In worst case scenario (if they reject), you would loose Travisa fee.
