Name switched around

I have to deal with a very unique situation. My wife's Indian passport has her name switched around first as last and last as first, which we did not notice even when the passport was renewed second time at chicago consulate in 2009 based on Green card. She has travelled to India with green card and the same passport. Name on US passport is fine.

Now I received a email from travisa stating that the name change document evidence duly notarized to be submitted within 10 days. I did speak to them twice, both the times the ladies said to submit a affidavit stating during naturalization the name got changed. Which is not correct and do not like to give a wrong written statement. Instead I am trying to explain how the name got switched around and blaming ourselves due to the confusion the name got switched moment we landed in this country. In notarized affidavit saying I am saying that I am the same person none other than mentioned in the OCI application.

So the question is will they accept my affidavit, if not there is no other documents to produce as there is no actual change took place.

Would appreciate insight on my problem.:(
 
I have no experience in this matter, but would think that all Travisa is looking for an affidavit and tick off that the document was provided. How you play around with the words is probably of no interest to them. You should start off with a template that either they provided or used by someone else and tweak the words accordingly.

Might also help if you drop-off your OCI application in-person.

from what i see in a pure bureaucratic perspective, it is a name change. She should have fixed it when she first got the passport (or filled in the application).
it is good that they are not asking for a court certified name change document.
All she has to say is that
My name was X Y
It was written in Indian passport as Y X
During naturalization it got changed to X Y
Please continue to use the name X Y
I do not see whats the issue with that...
 
We are doing an affidavit stating that this is my name X Y initially. Now it stands and I go by Y X. There is no name change as such. That is all I can think of and cannot create a document and hope for the best.
