Name Check question

Is the Name check only done for a primary applicant? In my fax request I only included my case details. Should I have included all my dependant's info also? Please let me know.

Name check is done for each individual case and the applicant should be above 17 years of age ( I am not so sure about 17 or 18 ). So give details of all dependents if they are above 17 years of age.

Name check is done for all adjustees who files for I-485.
