Name check: Email Address and Fax#, please

I know that there are some threads that have this information but couldn't find them using the search tool. Could someone please give me the email address and also the fax#, along with the template?

Is one method more effective than the other?
 
This is the fax no and email address for Name Check. Now a days email is working. They respond after about one month. I also applied for second EAD but have not received the receipt. I hope something good is going on there.
