Name change

I would like to change my name-Has this to be done seperately? Or can I indicate it in the form and that is it?

Just on the form, USCIS will do it for you.

Note it on the form. You will be asked to sign a 'name change' form at the time of interview. Then at oath time, your certificate of naturalization will reflect your new name, and you will also receive that signed 'name change' form you signed at the time of your interview.
