Name Change Record

When my girl friend apply for citizenship, she chose to have her name changed on the application. She did not remember having to fill out other form(s) related to the name change. When she finally received her citizenship certificate, the new name (that she picked) was on the certificate. Now applying for passport for her children, she was asked (by passport office) to submit a copy of "Petition for Name Change" form that they said would have been a part of the naturalization process and executed (by a judge) at the time of the oath ceremony. She had gone to the BCIS office to get a copy of the form, and was told to go to the county court house. She went to the county court house looking to get a copy of the name change. But, they (the county district clerk office) could not even find record of her name change. And, they did not think that the (INS') "Petition for Name Change" form would have been executed and filed with the county court. Has anybody had similar experience or heard of anything like this? Does anybody know which judge (county, state, federal) normally administer the oath ceremony? Where can she find a copy of this name change order?

For naturalization purpose, the name is changed at the federal district court. She should have received a copy of the name change court document along with the certificate. If not, find out which district court her local USCIS bolongs to, or go to USCIS on an Infopass and ask about it.
