Name change on the passport issue

Hi,

I would like to purchase a ticket for the US for a month. I was wondering about the following:

I have my passport, Which is valid and i have my old name on it "John Doe" now i went ahead and changed my name to "John Smith" a while ago, I changed it on all my internal Israeli documents and ID's including drivers license, But i did not change the actual passport, What i got was when i went to the Israeli ministry of interior and they just stamped this piece of document onto my existing passport which specifies that i changed my name and shows the old and new name.

I was wondering first, On which name should i purchase the ticket on? Second, Will i have any sort of problem when entering the US since my visa is signed with the old name?

P.S The names are just an example. I am not a US citizen, Was traveling to the US a few times and never overstayed. I have a valid visa.
