name change during interview

1) I got my name change during my interview, last name is still the same, first name is difference, and i also got a form called petition for name change which listed my old and new name. does anyone change their name during an interview have that form? Do i need to bring that form with me to the oath ceremony?

2) will i get an "official certification for name change" during oath ceremony together with "certification of citizenship"

3) what should i do after that?

My understanding is, if you request name change when filling N400, you are queued for Judicial oath.

Did you request for name change when filling N400 or decided to do name change while at interview?
Did you have to fill any forms for name change or just asked IO to change name?

i requested a name change during the interview, so the IO printed out a form which change my old name to new name and i have to sign it, i got to keep a copy of the form. I did not request name change while filling the N400. I think he told me that i will take my oath in the court house.
