I tried to get an answer and could not find it exactly.

If a traffic ticket was received and the fine was over $500, should all the following questions be answered as "Yes"

15. Have you ever committed a crime or offense for which you were not arrested?
16. Have you ever been arrested, cited or detained by any law enforcement official?
17. Have you ever been charged with committing any crime or offense?
18. Have you ever been convicted of a crime or offense?

or "Yes" to only Q-16a

please advice.
 
The ticket itself (irrespective of the amount of the fine) only causes Q16 to go Yes. The $500 cutoff is for whether you need to provide an official, certified proof of disposition or whether USCIS will allow you to self-certify that you took care of the ticket.

The answers to 17 & 18 will depend on the 'type' of ticket and how it was resolved. E.g if you were charged with DUI but not convicted (yes, it happens) then 17 will be Yes and 18 No.
