N400 Employment question

Dear All,

I am applying for citizenship today. I am a schedule A nurse, got my green card through an agency in NY. I was on contract with the agency for 1 yr. But the agency put me in client hospital to work
My I-140 was sponsored by the agency, and i worked at a different hospital for the agency.

On the N400 application Part 6 asks for employment history.

Now do i write the name of the Sponsoring agency or the hospital where i worked for the agency?

My pay stubs are from the agency and not the hopsital.

Your employer usually is the one who writes your paychecks and provides W2/1099.
Since your GC was sponsored by the agency, and the pay stubs are also from agency, it is a clear case of your employer being the agency.
