N400 Application Question

I received a public urination citation in 2002. I have proof of payment etc. Do I include this proof with the N400 or do I carry proof to the actual interview? Also, I called the appropriate authorities in Chicago and the best they can do is provide a screen shot of the citation, proof of payment etc. and mail it to me. Will that suffice or does everything need to be notarized or anything of the sort?

Yes, you need to include that on your application. Screen shot of a citation and proof of payment is sufficient.
