N-400 related questions... Need help.

1. One of the Civics question asks 'Who is the head of your local government'?. Is this question asking about the Mayor of my current city? If not, what is it?

2. Does anyone has an experience of filing a FOIPA request when filing N-400 so that if they are stuck with the Name Check formality after Interview, it reduces the time for the USCIS to do the checks?

3. Anyone recently applied for N-400/got interviewed from Portland, Oregon?

4. I remember a Traffic speeding ticket (less than $100 paid in fine) that I got sometime in the Year 1998. The Driving record of the State where I got the ticket, did not show up the ticket in their history. So, I called the concerned District court of that State to find out if they have the ticket dates, fine paid etc. They also didn't have the information of the ticket I got in Year 1998 but sent a letter to me in their Original letterhead (after my request in the phone) that any records which is that old are deleted from their history and that there are no records in my name (with all my given details).

Now, my question is do I need to mention this ticket in Part 10 D. of N 400? If I need to mention, how do I mention it since I don't know the date on which I got this ticket and I also don't have the Citation given by the cop?

5. How to sign in 'Part 11. Your Signature' of N-400? Is it our full name to be printed without cursive style OR do we need to sign our full name in cursive style? If none of these, how to sign correctly?

In most places, yes, the mayor is the head of the local government (it can be a little weird in some places in New England) - find out who is your mayor, your governor, your congressman, your senators, your state representative and your state senator - it's all good to know at the interview (oh, and the president, vp and head of the surpreme court).
