8 a. Have you ever been a member of or associated with any organization, association, fund foundation, party, club, society, or similar group in the united states or in any other place?
for this one, I used to join USANA From May 2008 to Feb 2010 and also used to join something like American Cancer Society in 2007 for one year which I paid $100, I didnt pay again and didnt renew. Do I need disclose these? But I am wondering if this counts, how about COSTCO and SAM's Club? and 24 Hour Fitness? Any guru can share some insights to this? Thanks a lot!!
