OK, this could be a little confusing, so please stay with me - any help is appreciated.

From 1999 to 2005, I was employed at Company A. Couple of months before the end of my employment there, Company B acquired Company A and laid a bunch of people off, myself included. So in the employment history, I have mentioned the following

Company B (formerly Company A)

Is this correct? Also Company B moved the corporate headquarters to MA. But I was still employed in Company A's CA office all the way until the layoff. Now Company B does have an office in CA according to their website, but it's a different location than the address I was at. What should I put in for the address.

Although one cannot know for sure, it is generlly admitted that the employment hstory part of the firm serves at least two purposes:
- allow the USCIS to check the applicant's history, in particular for employment-based Green Cards
- allow the USCIS to make some background checks at the place of employment

Once a company such as Company A is acquired, the new owner often changes all letterheads and logos to reflect the new company name such as Comcasny B. Consequently, it is in my non-professional opinion a wise choice to enter Company B (formerly Company A). The reason is that if you are asked to provide additional evidence, the best you can get is letters of employment with Company B's letterhead, not Company A's.

If you feel like being a little proactive, you may get a letter of employment from your current employer (Company B) if you wish, and if you do so you may also get some publicly available information about the merger - unless the merger is so publicly known that everyone would know, for example AT&T and Cingular.

The physical address where you worked. I would not recommend mentioning the company's headquarters unless you specifically worked at the headquarters. I believe it makes sense for the following reason: suppose hypothetically you work at a McDonald's restaurant in CA, if you mentioned McDonald's headquarters in Oak Brook IL, the USCIS would legitimately wonder how in the world you commuted every workday from CA to IL ust to work in a restaurant. :)
Just my 2 cents.

Thanks for taking the time to reply. I'll make sure I get some printouts of the news items about the merger in case they ask me about it.

I had the same issue as well. I have been with the same company which has been acquired and has under gone a name change a whopping total 5 times! So for that question on the N-400, I simply listed the new company name and obtained an employment verification letter with my original hire date. I am going to take a copy of my resume to the interview in case the IO asks any questions.
