N-400 Filling Up The Name And Name As Appear Exactly In Your Lpr

I kinda get confuse about filling up N-400 Part 1. B:
My LPR name say REYES MCCORMAC, DALISAY.

But my real name is Dalisay Reyes McCormac (ID, SSN,and when I get my LPR card the name outside at the delivery address says Dalisay Reyes McCormac). Reyes is my middle name.

How should I fill up the form? (MCCORMAC, DALISAY REYES OR REYES MCCORMAC, DALISAY). Any advice? I will submit my N-400 this coming monday. I would appreciate any input.
 
Under 1B of the N-400 it says ..

Family Name (Last Name) .. so insert your Last Name as it is on your GC and then it says Given Name (First Name) .. so insert your First Name as it appears on your CG and the same with the Middle Name if you have one.
 
Thank you very much lady bug, i appreciate your input on this, I'm going to fill it out as it appear on the green card. It's just hard to accept that I would have to do a name change later. I know changing your name takes forever.
