1) Do you need to mention every address where you lived, even the duration is less than 30 days?
2) What are the consequences of failing to file AR-11?

If you stayed somewhere for a few weeks while maintaining a primary residence elsewhere, that's just a visit and you don't have to report it on the N-400.

But if you stayed for a few weeks at a place that was your only place to live during that time, you should list it.

What are the consequences of failing to file AR-11?
Just file the AR-11 ASAP. They're not going to go after you for past failures to file it, as long as you file it for the latest address.

1) If you used that address on tax return, I would suggest mention it.
2) No need to file AR-11 for past addresses. If you missed to file AR-11 for your current address, it is good idea to file AR-11.
