Is it OK if I sign the forms on his behalf. I just signed his name and I did not fill anything in the place of person preparing form. Is that the way???
 
How old is he? I am not sure if signing his name on his behalf is a good idea. Then again, since I have not done it, I shall let others answer.

PS: I think you need to sign as "preparer"
 
For my children (9 and 10), I signed my name on their form and put *Father* or *parent* beside my signature so that there was no misunderstandings. It was approved in this manner.
I don't believe that you should sign their name, as it would not be a legal signature.

Inspite of my mistake (signed my son's name), today I received his notice of action (Receipt). May be it works. He is nine years old.
 
I doubt it would be a problem.
If it is you will find out when an officer actually reviews the case, not when the file is received.
