My Middle name change after marriage

My Middle name changed after marriage . Does it constitute a Change of Name in the application N400 . My Birth certificate does not have middle name but High School does have and after marriage the name on the passport (middle name changed ) . What should I be doing in the application (Giving Birth Name , Name on High School Certificate and Current Name as three different Names - 2 with Middle names and one without?
