Please confirm that the following birth certificate is what other people have gotten, and it is ok:

From:
BRIHANMUMBAI MAHANAGARPALIKA
Public Health Department

CERTIFICATE OF BIRTH

Has name, date if birth, parents names; but has the following issues

Name: >
Name of mother: >
Nationality of father/mother: NOT STATED

Is this ok???
Other people born in Mumbai, got similar certificates? Do these work fine
 
I don\'t think it would be a problem

In any case, for peace of mind u can get 2 affidavits done for your birth date, which will be additional documentation..
 
I have excatly the same format ..my last name is misspelled..what do i have to do...

I have Caste or Nationality column..in which i have my last name which is misspelled...i guess i have to get affidivit ...can anyone give me the format and how am i suppose to do it...

My birth certificate is the same way from mumbai has only first name & nationality of father not stated but mother & dad\'s name has both first & last name.Hope this helps
