Anybody has successfully applied multi-entry visa from Canada to USA as a canadian permenent resident? Will you please share the procedures of how to apply this type visa?

What kind of visa are you talking about (H1, B1/B2, L1/L2)? Also you are posting in the wrong forum.
 
May you forgive me for posting this question at a wrong place. As you can see I am still a junior member.
I am talking about H1-B visa

What is your situation? Are you a first time H1 visa applicant from Canada? Are you already in the US on H1 and want to get a renewed visa? Are you in US on another visa and now changed status to H1 and need a visa? It helps if you post such details.

Do you already have a H1B visa?

If you already have an expired or soon to be expired H1B visa in your passport that it would be much easier/safer for you to apply for a renewal of visa at the Department of State. This way you won't have to travel outside the US just for the purpose of getting a visa stamp.

However if that is not the case, then you would have to schedule a visa appointment at any of the US consulates either in Canada or Mexico. Typically you should apply for an appointment a month in advance.

I appreciate so much for your info. My H1-B visa will expire within 2 months. If I revalidate through Department of State I only will have a single entry visa. I heard Canadian PR can apply for multi-entry visa to the US. Do you know how to do it?

PROCEDURE FOR APPLYING FOR VISAS AT THE U.S. CONSULATE IN TORONTO
The U.S. Embassy in Toronto is located at:

To apply for a visa from the U.S. Consulate in Toronto, you will need to schedule an appointment in advance. Appointments must be made through an automated number (either 1-900-443-3131 or 1-888-840-0032), or through the following website: http://www.nvars.com. If you call the automated number, you will be led through a number of prompts including (1) entering credit card number (to make an appointment, the Consulate charges for the call), and (2) entering passport number. After having entered this initial information, you will be given four options: (1) review an appointment, (2) reserve an appointment, (3) listen to general information or (4) speak with an operator. You may want to speak with the operator and ask for information about scheduling an appointment at the U.S. Consulate in Toronto. The operator will instruct on how to set up an appointment and what documents you must take to the appointment.

Applicants who make an appointment will be mailed an application package. The visa application package includes the visa application forms, an information sheet, and a pre-coded deposit slip for the non-refundable US$100.00 processing fee. The fee must be paid at any branch of the Bank of Nova Scotia (ScotiaBank) located in the province of Ontario before coming to the Consulate. You must present the last three validated copies of the deposit slip at the time of interview. You will also need to bring one photo per application form, taken against a light or white background.

CHECKLIST FOR VISA APPLICATION
You can use the following list as a checklist to confirm that you have gathered all of the necessary documents for the visa application. Although you should verify these items with the U.S. Consulate in Toronto when you make a visa appointment, you will likely need to provide the following documents to the Consulate:

Original INS Approval Notice (Form I-797)
Copy of H-1B Petition
Passport: You will need to present a current passport. You should also be prepared to present any expired passports that contain U.S. visas.
Visa Application Form: You will need to submit one completed Nonimmigrant Visa Application (DS-156), which is available online at http://www.travel.state.gov/DS-0156.pdf.
Supplemental Visa Application Form: All males between the ages of 16 and 45 need to complete the Supplementary Form (Form DS-157). It may be obtained online at http://www.travel.state.gov/DS-0157.pdf
Photograph: You will need to submit one recent passport-sized (5 cm x 5 cm) black and white or color photograph with white or light-colored background. Please note that the individual in the photograph must be looking directly at the camera, and the face should fill 50% of the photo.
Visa Fee Receipt and Reciprocity Fee: You will need to present the receipt from Bank of Nova Scotia/ScotiaBank) for the $100 visa fee. After arriving at the Consulate, you will also be required to pay a $75.00 reciprocity fee, which may be paid in cash at the Consulate.
