There are no forms to be completed. I came across with a U-Haul. it was the only rental truck company that allowed cross border. absolutely no hassle from the border officers.
If you hire the moving company, then yes, they will give you some forms to fill and to sign. Also, if you are moving your car using the moving company, then you need the car related forms too.
I had moved my stuff through Atlas Van Lines (only the household items). They were very good.
Just be sure you have a list of items in case they want to search. Have them numbered and everything. Better than having them go through everything inside/out.
