I have a traffic violation (reckless driving). The disposition was withheld adjudication and I had to pay about $100. What should I answer on the form? There are two questions - Were you convicted (yes to this) or Were you placed in an alternative sentencing program (e.g ...., withheld adjudication, ....).

It can't be an indefinite withold of adjudication. What is the current status? Normally it is witheld pending something you have to do, e.g., take a driver's ed class. Once you take the class, they dismiss the ticket (if that is what the court ruled). If all you had to do was pay the fine, then the judge must have entered a guilty verdict. You should go to the courthouse and get a current copy of the disposition. It usually takes a few minutes for them to locate it and make a copy. Usually it is obtained at no charge. You will need a copy of it anyway for your application.
