Need Help

I called USCIS regarding my spouse 2nd FP notice (as I have received mine and completed 2nd FP on 18 dec 2003) and the lady from tire 2 advised me to send a letter to Service Center requesting 2nd FP notice.

Is there any specific format to send letter to service center ? Please give me a format if somebody has one...
