Hi All:

While filing I-485, what kind of medical tests reports we need to submit. Please advise us or give some site where I can find this information. Thanks.
 
There is a specific USCIS form, I-693, that you need to take to a USCIS designated doctor in your area (called a "civil surgeon") to fill out. The doctor will give you a medical test and perform some vaccinations or evaluate your previous vaccination record, and will fill the form accordingly. The doctor will then seal the form and give it back to you, and you must include this sealed envelope with your application. Follow the instructions here.

Also, this page will help you find a "civil surgeon" in your area. Some smaller states have very few of them. You may need to call and make an appointment. You will usually need to make 2 trips to the office.
 
Bring your vaccination records when you go to see the doctor for the green card medical. The doctor will look at the records and reduce the number of new vaccinations based on the old ones shown in your records.
