Medical Related RFE Question

I've just received a RFE:

"Please return to your civil surgeon for the completion of varicella vaccination. Also show evidence that Td series was completed."

Here's my situation:

I took medical examination in 2002. During the visit, I took 1 Td shot. The civil surgeon suggested that I take the 2nd shot 4-8 weeks later and the 3rd shot 6-12 month later and wrote that down on the form submitted to BCIS.

Unfortunately, I did not take the shots according to the suggested schedule. Instead, I took the 2nd shot in 2003 and just completed my 3rd shot after getting the RFE.

I did some searches online and found out that according to CDC, if the sequence of the shots were broken, there is no need to restart.

Now I have a few questions and your answers/insights are greatly appreciated:

1. Have you had similar experience or heard of something similar happened? How did you deal with the situation and what was the result?

2. Is that possible that BCIS makes an unfavorable decision because I did not finish the shots according to the schedule suggested by the civil surgeon?

3. How would you answer the RFE? When replying to the RFE, would you explain that CDC does not require the restart of the series if the sequence is broken?

4. Since my civil surgeon does not provide varicella vaccination, so I completed the shots at a medical center nearby. Is this ok? Should I explain to the BCIS the reason that I took the shots at a place other than my civil surgeon?

I had received the same RFE, just went to the Dr. office and took the shot. Sent back the RFE with documents from Dr. office, and got approved.
