Answer: P4 is packet 4 which contains your appointment letter for your immigrant visa interview/ GC interview. This is issued by the consulates only after completion of PAcket 3 review eitehr by NVC in a traditional CP process or by the consulate in an AC I-140 process.

Normally one needs to go only to those doctors listed by the USA consulate and not to a family physician/ family GP. The list will be available on London USA Consualte web site and it will explain what is involved in the medical examination.
