Friends,

I want help for a friend of mine. He wants to know, how to get the form required for the Medical before applying for I-485? I think I got it from my lawyer, but he says his lawyer does not have the form? Even he checked with his doctor's office and they also do not have it. Where else can we get this form?

you will get name and address of doctors (registered for INS medical) living near by you.

Then call them... have appointment and get medical done.
You are paying them they should have forms related to INS. If not select another doctor.
