Your I-485 will NOT be returned without the medical. They will issue you an RFE for the medicals. Thats what we did. It doesn't really hold your case up to long. It's more important to get the I-485 in before October 1rst if you are affected by retrogression. At least then you can apply for EAD and AP. If you don't have a filed I-485 you CAN NOT get these benefits.
