My lawyer sent me I-693 forms for the medical evaluation. These are not original (blue forms) but a photocoy of the same. Somebody told me that the medical practioner usually insists on the original which is available through the INS office. Is this true? For people who have gone through the procedure pls advise.
 
Check with the physician's office if they would accept photocopies. If not, pick up the form from your local INS office or order them by mail from ins.gov
 
Most doctors have their own forms

My doctor did not use the form I printed from INS website. He has his new form which he wrote only once and a few copies were done. he said it's a kind of new paper (of course the same form).
 
Check with your Physician

Most docs will have the forms. Some may ask you to bring the forms along with you. Check with them. If you have to take it, pick it up from the local INS office or directly from INS by mail. You may also check in your local public library. Sometimes they carry some forms. Check with them as well.
