Hi guys, I just became a US Citizen last Friday, and I am preparing my wife's application for status adjustment, however, I am not really clear on the medical examination part of it (Form I-693). Is it only required for her? or I am required to take and submit one as well?
Also, she lost her I-94 and we need to request a duplicate one, however, in the form they ask you for the form's number, but, since she lost it, she does not remember it. Is it a problem if we leave that part blank? or is there a way to get that number?
