Hi All! I am from India and planning to apply for I 140 & I-485 app along with my wife.
Questions.

1. I am going for Medical Examination. What type of supporting docs should I carry with me to avoid extra vaccination required by the exam ?
2. Should I get a vaccination card from India? if so can someone tell me the format?

You can get your blood tested for your immunity from your health care provider and can carry the same reports to the BCIS certified doctor. I did the samething few weeks back and I got my medical report.
