Medical examination

Hi,

I took medical exam (for my I485 application) a week ago and
I just got my sealed package today. I was also given copies of
the forms in the sealed envelope - in which I found that the Doctor
left the "passport number" space empty on the supplement form
and the A# empty on the medical exam form. I was told by my lawyer
that I have to make sure that the doctor does not omit any form
or leave any space on the form empty, so I called the doctor
and doctor told me they never fill in passport number and A#.
At the same time, one of my friend said his just got his doctor
fill in his passport number..

Question:
Is it an issue for INS if the doctor does not write down your
passport number in the Med Exam supplement? Shall I ask the doctor
to fill in it anyway? ( The doctor claimed that he never did it
before and he is not willing to do so for me).
Please help and comment.

Don\'t worry about that. Most of the doctors don\'t fill that in and when I asked my doctor, he said it does not bother INS.
