4mm is fine. The official CDC limit is 10mm. My wife had 13mm because a stupid afro-american nurse gave her double dose. Anyhow, she had to conduct X-ray and it showed negative results. In your case, I won't even worry.:)
 
TB skin test / x-ray

I had 12mm X 12mm measurement for my tb skin test. I was required to take an x-ray based on the results. x-ray results came out negative (also known as 'false positive'). So, everything was normal in my case.

If your doctor didn't require you to take x-ray based on 4mm measurements then you are ok. even if you had to take the x-ray ; so long as the results were negative then you are ok. most people from india get positive reaction for skin test and are required to take x-ray to re-confirm the test. But dont worry about just the measurements. but if your x-ray came out positive then you will be required to take medications for about a year.
 
Doctor wrote 20 mm for me on I-693. Does it affect in Future

The purpose of test is know whether some one has TB.

If TB Skin test is negative, no TB. You are good.

If TB Skin test is positive, X-Ray negative, no TB. You are good. The size of the circle doesn't matter.

If TB Skin test is positive, X-Ray is positive, you have TB. You need to worry more about your health than GC. Treatment may take 9 months. I have no idea what happens to I485 though, which depends whether doctor hands over medical exam report to you are not, RFEs and so on.

Good news is, I am yet to hear that some one is diagnosed as having TB. So fear not.
