Medical Exam. questions?

where to get Supplement Form for I-693 Form (medican exam.)? Does the Doctor required to fill that form?

Also, there are 4 copies of I-693 form, does this mean the applicant can get the exact copy as the one sealed and sent to INS?

You usually get the form from the doctor

There is one on the INS webpage, but the doctor\'s copy has
yellow/pink/white copies and my doctor refused the downloaded
one I offered. (I\'d take one just in case).

You must present the copy in a sealed envelope with your
application. Some doctors supply a copy just in case the INS
loses your forms.
