I'm filing AOS for my mother who is on US with a B2 visa. Anyone has any experience to share, good/bad civil surgeon in NYC area?
How much you paid? Do they got all shots again (MMR, etc)? How long you receive the envelop back?
Thanks

Hi,

there is a civil surgeon list on the USCIS website, those that are accredited by them, if you put your zip code you should get one close by you. And i know they charge about $300.00, she will get all the vaccination if there is a need to and also TB skin test etc, they then give you a medical examination and immunisation result of the i-639 form sealed which you must not open, and you send it along with your other forms. Make sure you do teh medical before sending the forms because it will result in a slight delay of your application for her such as her change of status i-485, since she is here on a B@ visa , do this before her i-94 expires. Make sure you use the surgeon designated by USCIS from the list on there site.

Denucci,
Thanks. I got the list of surgeon and I called around. The ones near my place is charging around 300 with vaccination. The ones in Chinatown charge $180 with vaccination. They told me it will be blood test, MMR and TB test. One place tried to convince me to do Hepatitis B for $200 extra. They told me the medical exam may get rejected without that hepatitis B. I think the vaccinations are done for different age group.

According to the I-693 instruction:
By law, the required vaccines include: mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, influenza, hepatitis B, haemophilus influenzae type B, varicella, pneumococcal, rotavirus, hepatitis A,meningococcal, human papillomavirus, zoster, and any other vaccinations recommended by the Advisory Committee for Immunization Practices (ACIP).

Go with a designated physician. You may not get a second chance with this. My physician also charged me around 300 for each of my parent. You get the envelope a day after the TB skin test is cleared.
